If you need to be granted the right to affix the mark of conformity CE on the personal protective equipment you manufacture or sell, covered by the PRS S.A. notification, you shall:
choose the module, thus the conformity assessment procedure for your product,
fill in  Application Form (87kB) (only for breathing devices of diving equipment).

After receiving the completed Application Form, we will send you our price
offer and Contract for Product Certification for signing.
Through signing the Contract for Product Certification an agreement
for the given product certification is concluded. After receipt of the order we
will determine the range of required documentation of product and we will
specify details referring to the tests.
