define the design category to be requested,
choose the product module, thus the conformity assessment procedure for your
product,

Form (40kB).
After completion and sending back the Form, you will receive our price offer
and Product Certification Request for signing.
Through signing the Product Certification Request an agreement for the
given product certification is concluded. We will then initiate without delay
agreeing details concerning assessment of the design and structure, examinations
and tests. If required, we will order conducting laboratory examinations in
accredited or approved laboratories, with which we concluded appropriate
agreements.
